Appointment inquiry composer

The current website prototype does not submit your appointment information to a website server. Name, phone number, service and preferred schedule are used in your browser to prepare a text message for you to copy and send yourself through Facebook Messenger. Information is not stored in a site database by this draft.

Facebook Messenger and external links

When you open Messenger or other third-party websites, information you choose to submit is subject to their privacy practices. For information on Meta's privacy practices, visit Meta's Privacy Policy.

Sensitive health information

Do not include diagnoses, test results, medical history, or other sensitive medical information in the appointment request composer. Contact CDC directly if you need guidance about what information to provide.
